Can I turn Sacred items into Sacred Uniques?

Also How many MOs can I put into a honorific item?
I've got a T5 belt with rlvl 39 and *20 required level from orbs which should make a total of 59?
I understand that MOs are limited to lvl 60 and below?

Does item tier matter when making a unique item?
For example if I turn a T6 or Sacred armor into a unique will I get a T6/Sacred unique or T1?
