1. It's always been this way, even in vanilla D2. If there are no other gems in your inventory, or only p-gems, Gem Shrine drops random chipped gem.
2. It's D2 bug rather than MXL bug. Similar thing sometimes happens in A5 in which killing Shenk the Overseer doesn't result in quest completion. Either remake the game or reroll the map and try again.

Not a bug, hover your mouse over lvl18 blood skill and read the text written in grey color.

Also, what maphack you're using?

I use Mmap 0.4 , AND yes i know Devotion skill but i don't put ANY point on any tree , just like u see , at lv18

What "Devotion" means is that u dont put points in any other tree at all, not just the lvl18 skill, so if u wanna put points in lvl18 skill then u should respec and put points in only 1 tree.
