Glide wrapper problem / no 3dfx mode

Hello, i have problems with Glide wrapper.

I did install Median, but mod starts only in DirectDraw and Direct3D video mode.
If i try to start from Launcher in Glide 3dfx graphic mode i get ACCESS_VIOLATION (c0000005) or launcher just reopens without game even trying to start.
I tried all 4 Data Execution Prevention methods - didnt help.
I did reinstall Diablo2 and Median, cleaned reg and folders - didnt help.

glide-init.exe build-in test crashes and shows that wrapper stopped working.

D2 graphic test doesn't show Glide mode and logs show that GlideDetect failed:

I did test my OpenGL and run some test - seems to work just fine (ver 3.0 to 4.5).

did you play around with the glide warpper settings?
the normal glide is not supported by newer video crads, so we use the workaround that is built in the launcher.

post the settings of the glide wrapper and make sure to disable "keep desktop composition".
plus there is a median launcher logfile, find it and post it

PS.
After reading this log i realized that i didn't clean ~\AppData\Local\Programs\medianxllauncher before reinstall, so will try to reinstall one more time and clean everything before that.

EDIT:

~\AppData\Local\Programs\medianxllauncher is deleted during uninstallation, so nothing to clean. Reinstall didn't help. MOD works only in DirectDraw and Direct3D video mode.
