Can a Shimmering circlet really get better than a T4 Glowskull? I'm trying to upgrade from the low to mid level gear, but I don't really think it makes sense for the helm.

Looks to me like you can add some upgrade to a crafted item. How do you do that?

I see there are two recipes for upgrading uniques: >Unique non-sacred weapon/armor + Arcane Crystal + rune → reroll as next higher tier >1x Arcane Crystal + Item (Tier 1-3) = Item (Tier +1) I realize now that I have always been using the 2nd one.
