Can someone explain the Oil of Luck and the "Already Upgraded" mechanic in some detail?
e.g. when does the game decide the item get the Lucky bonus or not - when actually transmuting the item or from the time the item is found/spawned in the world?

When the item is found/identified. So nothing can change the outcome of the "lucky" bonus except a complete reroll. Oil of Luck, Oil of Conjuration and the Lottery cube recipes all add "Already Upgraded!", and they are mutually exclusive.
