I was just testing this, and the lowest I've gotten is lvl 23 req, so no.

You cant get two procs on same item with shrine crafting because all procs are from same affix group 223 which you can check here if are curious.

If you saw more than one proc on shrine crafted item of other person then it came from UMO.

The only monsters that fit what you're asking about are the veteran monsters, which are found near the end of each act.
