Because you hit so many times, poison doesn't seem to be that significant. Wdm of maelstrorm is pretty high which is good for poison, but if you deal multiple times more damage via other types, as poison would drain over 1s, it imo isn't worth it.

Doom is totally worth it especially in the early game when it gives you a relatively huge damage spike. Later on casting it can become too much of a hassle since your damage is more than enough.

I don't know about that. Now you get magic damage to each spike, and that number is relatively high. I think it surpasses doom bonus damage fairly soon. Even at 1000 dex 100% total dex as magic * 80wdm this is already 800 magic damage per spike, a lot more than doom.
In endgame this gets even crazier, let's say you have hexward and pump maelstorm for 250% bonus, get 6k dex, that gets you 12k per hit.

I'm having hard time with mana since hell, i'm almost 110 with 20% mana leech + 200 on hit but i'm emply in like 4s

I got 40% mana leech and have no mana sustain problem even for ubers. And I heard Mana on hit is only works with melee skills.
Get more leech jewels and use that knife for pure phys proc. It worked very well at least for me :)

Mind if I ask what gear you used before Piranha Swarm? This char seems to want to boost dex for Maelstrom, but then most ppl seem to max Broadside as well which needs different stats. None of the TU throwing knives seem good for this build.

I got to level 65 Act 2 NM and it takes about a minute to kill the big ugly poison things that drop lots of gold in the Dry Hills. But I've been really unlucky in drops - only one rare throwing knife dropped and nothing else useful, so if I need to go farm crystals to make TUs I kinda need to know what I should be making :?

I'm pretty far with progression of my sin.
For weapons I went: Random rw -> Lo runeword -> Lo runeword in sacred knife -> tracker set knife -> Lo Xis in eth sacred knife

While you can sustain mana from knife procs, from either tracker set, su or craft, vs single target, bosses, bosses in rifts - not gonna be enough. Lo xis gives you enough mana sustain for bosses, good damage, (runeword ed and +150 damage average, 40 slows target, some reanimates).
Thing to note here is, knife procs are 50 wdm, no bonus magic damage. Maelstorm spikes are 80% wdm, converted to magic (-40% res from doom), and they get +magic damage bonus, which can be over 10k with high endgame dex and hexward shield. In other words this makes them way stronger than knife procs on gear. It is not same scaling as old builds that relied on ways damage and procs, ways are very weak in comparison to magic damage you get from maelstorm skill dex bonus.
You end up throwing maelstorm and each spike from maelstorm deals for example 3x more damage than those knife procs you see.
