how do mystic orbs work?

So im curious and I honestly don't know how. Like I feel like I know a little bit about the game after playing for weeks on ends but I still don't know everything. Like for 1 how in the world does this person get all this bloodlust percentage when the normal percentage is 60-100% bonus damage.

That is from an older patch, it's no longer like that and as you can see even the runeword is not made by socketing a Tal + Kab stone anymore, it's now an Enhanced Runeword, very similar but not the same. Also Qarak's % damage to Bloodlust was nerfed some time ago, so it's just up to 100% now on both. And i don't get what the title has to do with what you are asking.

I was confused on certain things. I thought it was a mystic orb thing that did it but I guess not. also what is Qarak's?
