It's hard to say exactly what is possible without detailed specification of what modding features the D2R team is providing. That said, a couple things to consider:
* Median circa the late-00s and early-10s did not actually rely on new DLLs at all. Laz developed it entirely via the simplified modding interface Diablo II's maintainers made available (various spreadsheets that could be read in 'direct text' mode or compressed into patch .mpq archives IIRC). Thus, many of the core features of the mod (such as custom skill trees, monsters, and items) obviously don't fundamentally rely on DLL injection at all, though perhaps the implementation has changed in the last decade. I remember this because, back in Ye Olde Days you just had to copy a 'patch.mpq' file to your D2 directory to play Median.
* Since modding support is a stated priority of the D2R team, and they've already promised expanded mod support, I'd hope/expect modding features for D2R to enable a superset of what's already possible in Diablo 2.
With that in mind, I'd be sort of surprised if a significant portion of Median couldn't be adapted for D2R (not directly ported), assuming they actually mean it when they say they support mods. Maybe there's some features that will have to be reworked/abandoned, and I'm sure it will take a lot of time, but history has already shown that many of Median's key innovations were achieved via tools Blizzard made (semi-)officially available.
ALL THAT SAID...I'm not a Median developer so I'm just making a guess based on some time spent making D2 mods/playing Median circa ~2007-2010 (plus an irrational faith in the people making D2R to do the right thing). It will be interesting to hear from the Median devs once more is known about D2R. Personally, the idea of playing some flavor of my favorite mod with shiny new graphics is pretty exciting.
