With werebear, if you get 507 total ias and 168 weapon ias you can reach 5 FPA. With 209 weapon ias 4FPA is possible.

266 wias - 3 FPA if you push with relics

He probably meant these changes:
Lifeshield -> Barkskin
Circle of Life -> Hunger

Still, the guide is mostly up to date as of 1.8 but lacks end-game content. An update in 2.0 will be great.
