There is no entrance. To get to higher levels you find level 2 riftstones in that level 1 area, it's random drop. Same thing for 3+.

Cain's TP in Tristram can be used as a regular TP. Seems like it's random and doesn't always happen. Game had no other players, (Please don't actually fix this.)

Yep, you cannot.

Have you tried A1 Rogue merc? Deals nice aoe damage that makes use of neph spirit very well, and bear stance keeps her alive most of the time. Ranger uses Arrowside which is a huge boost to single target damage.

How many cycles can you use ?

All cycles have a "+x required level" stat. How many cycles you can fit in is limited by your character level.
