He's supposed to have 200 physical/magic/fire/lightning/cold and 1000 poison resistance, i know this because i had used whist's MXLutility, and when over the pentagram it's -50 physical/magic/fire/lightning/cold resistance but he still has 750 poison resistance. I killed him with miasma multiple times.

I had similar error when I killed Baal and entered the Tyrael's END GAME portal on Destruction. I didn't have time to screenshot it, so not sure if it is the exact same error

But it only happened once.

I fixed it by going into my task manager, found three versions of D2SE running...

Closed those, then restarted my computer.

EDIT: you may need to do a full reinstall of the game.
