All in all you've given the perfect recipe to create an extremely unbalanced and boring game.
1 You're speaking for yourself and not all the players, someone actually like that sort of gameplay, plus it is often used for the sake of balance due to limitations of Diablo II itself.
2 What's the fun in something that's "even more OP"? It's fun to relegate the whole game experience in spamming one single attack that wipes out 100% of enemies? It's way more fun and deep to craft your own "OP" build based on your gameplay style, not on written numbers.
3 I absolutely disagree. That alone would cut out a huge slice of the game and player experience.
4 If you're referring to normal immunities you're dead wrong because that bring variation in enemies and player's strategy to deal with them. If you're referring to the "heroic shields" I agree with you, but it has already been explained that they can't be removed because of balance strictly connected with Diablo II limitation when using great numbers.
5 That would shorten some processes of the game but ultimately ruin the curve of improvement in the character experience (already a bit ruined by gift boxes, imho).

Gift Box should be filled with only one item at time and will be still too overpowered. The reason why the Gift Box with those Tiered Uniques is out there is to give players option to learn mechanics and pick any class/build they can imagine from the scratch. If five runs killing Andariel is too much effort for you to uprade TU of your choice, so I dont have more to say about it than its laziness. Ten more Andariel runs and you can made by yourself TU's not provided with your class Gift Box loot.

Answering point 1: Dont run, just walk. Learn defensive mechanics and keep your resistances maxed perfect gems. Its survival of the fittest so use all crowd control skills/oskills you can get.

Answering point 2: There is such term called balance.

Answering point 3: Runes drop ratio is more than fair. Have you ever heard about The Forgotten Tower and its boss? She drop runes.

Answering point 4: No reason to remove game mechanics which makes game tastier and more challenging.

Answering point 5: Loot drop ratio is improved by stacking Better Chance of Getting Magic Items. It works well, also some mobs got guaranteed loot.

Answering point 6: You got on Hatred overpowered charm called Character Orb till level 120 and Harkon Crystal.

What if xdemonessx is just young player and had right to still learn what life is about and whats important and whats not? I dont like ignoring people just because they watching world with pink glasses. Young person can be smart same time even if got some radical or unreasonable ideas. Thats why discussion is always win in long run. Also we answered to his ideas making same time less work for mods and developers.

I suspect what confused people is the fact that, the right hand or left hand of your character are irrelevant for determining which is your "main" hand. All that matters is which weapon you equipped first.

Quick test I ran: two honorific axes, one with 30% ctc Miasma the other with 30% ctc spike nova.
1) Put both weapons in inventory, char hands empty.
2) Now put Miasma in your char's left hand (becomes main), Spike in the right. Miasma procs, spike nova does not. Repeat 1), but now put Spike nova in the left hand first - only spike nova procs.
3)Afterwards repeat the 1), but first put Miasma in your char's right hand (becomes main), Spike on the left. Miasma procs, spike nova does not. Repeat 1) with Spike nova going in the right hand first - only spike nova procs.

Ditto for two swords and Iron Spiral.

Considering it was reported multiple times since different releases, this kind of stuff is already a feature rather than a bug.
Still, only these skills are able to proc from off-hand and not just on attack:
Sin: bloodbath, batstrike
Barb: bloodhatred, cautious strike (possibly)

IMO buffing deathgaze with offhand procs would be quite OP, it is one still one of the best farmers, both for single target and aoe.
Same goes for bloodhatred life on attack - allowing to get that much life on attack with deathgaze / iron spiral is very powerful. I think such stuff was possible in some previous patch but was fixed for obvious reasons.
But what really needs a buff is noctule, which is quite inferior to crucify in terms of wdm damage.

it wasn't reported multiple times since different releases since there have been only 2 patches, XV, where noone reported it and XVc which is the current version.

There are chances that offhand proccing come from a special skill function rather than from dual-weilding itself, so maybe it's actually not possible to make this work for skills that use missiles. If you think about it, the skills that properly dual-weild all use states (batstrike, BH, bloodbath) and none of them use missiles. Maybe there's a limitation.
