Last night I was surfing around mods and found a modified(!) mod, which I couldn't find the original by my researches. Please if someone has idea which mod is this?

What you show looks very familiar to Min-Max mod. Maybe it got some patches, but how it looks for me in my current old version:




So my advice - google for Diablo 2 MinMax mod and go from there.
