I believe this may be due to how D2 works rather than MXL. My memory of it is:
Iirc the game apparently equips items top-down, starting with the hat (and... middle-left-right priority??? not sure on how it deals with items on the same vertical level).
So you can get a problem even if the armour seemed to be fine the first time you equip it ok because you were wearing the belt when you equipped it. Then when you die, you go back for your corpse and find that the armour gets left in your corpse because the belt with 80STR isn't equipped when the game tries to equip the armour. It gets everything else ok, but unlike many newer games, the game doesn't re-try the armour.
Normally this is ok since you can make a new game - or re-join the same game if on TSW - and the corpse with the armour will be at your feet (as long as you're expecting it / notice it ofc!). There can be issues though if you stay in the old game and die too many times and never manage to recover (click on) that corpse with the armour in it. When you join again, or go to a new game, the game only 'looks back' at a certain number of corpses (I seem to remember 16?) so your armour could get left behind and is lost forever.
