You need to edit the itemlevel line in the plugy.ini located in /MODS/median xl ultimative/. Then save the .ini file and it works perfectly fine just with the default plugy that comes with D2SE.
E: if it still doesn't work try these troubleshooting steps:
1) Set "run as administrator" option to enabled in the properties of game.exe and d2se.exe
2) If it still doesn't work a reinstall will probably fix it and make sure to be running the 1.13c patch and using the integrated plugy.

I feel like that should clear it up, but i'm derpy.
So, hypothetically, if I use Gorefest with a 1/2 weapon damage ratio, and decide to get 100% enhanced weapon damage, does Gorefest turn into 100% weapon damage, up from half?

ED multiplies your base damage.
Every distinct source of EWD is a separate additive multiplier to your weapon damage.
Skill fraction modifiers apply at the end of the formula.
Quick and dirty example: You have a sword with 5-10 base dmg, it has 100% ED. You also have 100% bonus damage (EWD) from strength and 50% from bloodlust proc somewhere. You use Gorefest, how much dmg do you do per hit? (remember some attacks multihit)
- Work out your weapon dmg: (5-10)*2 = 10 - 20
- Work out individual EWD bonuses: 1) Strength bonus = 1*(10-20) = 10-20; 2) Bloodlust bonus = 0.5(10-20) = 5 - 10
- Sum everything to get your normal attack damage: (10-20) + (10-20) + (5-10) = (25-50)
- Apply skill modifier to get final damage: 0.5*(25-50) = (12-25) per Gorefest hit.
