Unless a quest focuses on acquiring certain things and tell you to go into the vanilla locations, odds are it's likely remodified to be end game content - more specifically for those in destruction. The documentation covers it more in detail in the quest bar.
2) can't help you I use win 7
3) the documentation covers a lot of info you will look for but be forewarned- it's information overload and it'll take time still to get into the swing of things
4) it's rare for the tiered rares to have value that aren't rings and amulets. I'm not sure on the exact determination but to circumvent this issue, you can stash runes and put them into socketed throwing knives and axes and sell them for much more. The higher the rune, and the higher the act you are selling it in, the higher the gold return. You can also substitute jewels in the place of runes but I don't know what determined the value of jewel sockets piece.
Hope this helps, mobile may have raped the message I'm trying to give.
1. expanding on this - stick to the core areas during the levelling process (outdoor areas, level 1 of vanilla quest dungeons {i.e. do NOT go into the underground passage level 2, as this has actually been replaced with an uberlevel; same with many of the non-critical dungeons, like cave/hole/pit level 2 are all replaced with ubers}). this is due to an engine limitation of diablo 2 itself, preventing modmakers from creating entirely new zones, forcing them to replace existing areas (this is actually no longer an issue, and will be "fixed" with the next major iteration of Median XL, Version Sigma)
2. Glide is your most likely culprit, as MedianXL is VERY prominent with Glide, and even high-end machines will be affected - again, this is a D2 limitation, not your hardware. remember, Diablo 2 IS 17 years old..... (and this mod is still based on the core of D2)
3. same answer. read the documentation. consider MedianXL virtually a TOTAL new game, with only some aspects of d2 remaining. everything from skills to items to monsters, even the ENTIRE endgame (because really, there was virtually no endgame in classic, aside from "ubers" and Big Bad Diablo-clone) it is all revamped
4. because gold actually means something in this game (at least early game - endgame it's still easy to get), and because you should have to EARN your money

also, chucking runes in throwing weapons is no longer a thing. that got nerfed in the MXL2017 update. your best bet is magic rings/ammys, shitty rares, and apples
