i received help for my problem on the tech support in discord. Solution for the update loop: theres probably an old median verison running. get the new one (https://www.moddb.com/mods/medianxl/downloads), and install it via gettin the files in your diablo 2 ordner and probably overwrite the old ones...

Perhaps you should post your gear and setup so we can help you. melee sorc, full set, pretty crappy offparts but still able to do some quests, already farmed solo: heart of sin, rathma square, akarat, kurast 3000, creature of flame, death projector, butcher, infernal machi...
damn the one who invented the trojan jungle and put quests into it. i just wont continue playing just because of this "feature". Not being able to reach a quest, because you have to run a way longer than whole act 3, filled with mobs stronger than the quest is just hillarious. it was nice ...
