Think of it like "Adds X Damage" to what? If it doesn't specify then you should assume it's added to your weapon. It's a weird stat to have on the item but like others have said it comes from the runes themselves.

Necro totem build will get you to hell with zero gear, and it FLYS though normal and nightmare. It can do early ubers well enough with whatever gear you find but drops off at 115 in kurast without overhauling your gear.
