How to obtain?Start by obtaining the old
Book of Summoning. It will allow you to create portals to
Hardmode bosses (with additional modifiers), by consuming their boss charm. This is intended for trophy-only bosses, but can be extended to other areas.

Book of Summoning + Boss charm -> Hardmode Boss Scroll (single use)
Area modifiers: 15 sec lockdown, -200% flat elemental res, -25% physical res, more health & damage; bosses have life regen, soulstealer, immune to slow. Different effects make different fights harder (e.g. lockdown on Valthek/Astrogha, high purify uptime on Lazarus due to life regen).
Trophy drops: 6% chance to drop whole trophy, but you need to kill much harder boss. You can open instant portal in each game. If you fail, you go back to normal version to get a charm.
Quest: to get
Hardmode Charm, obtain 10 Hardmode fragments from Hardmode bosses (higher rate for high lvl boss).
Hardmode CharmCharm with debuff effects made usable for farming (magic find) and against Hardmode bosses (spell that reduces life regen of enemy). Outside mystic rifts you need to adapt your build to use the charm. The charm is requirement for mystic rifts, as the buff with charges counteracts debuffs and simulates time+death limit.

Holy Armor: only usable in mystic rifts, grants 2500 life regen, 200% elemental resists, 25% physical resists, thus counteracting some debuffs. Skill has 5 charges and buff lasts 10 minutes, thus simulating a time+death limit for an area. On death you will summon an enemy of your class.
Soul Drain: usable everywhere, cast on enemy to reduce its life regen, only useful if you can wear the Hardmode charm.
Limitations:
- Max 1 Hardmode charm per character (e.g. old median charms), to prevent using several of them in mystic rifts.
- Charm is ethereal, can only be recharged through recipe (swirly effect right when entering mystic rift)
- Effects of the holy armor buff should not be available if you remove the Hardmode Charm from your inventory.
Bonus quest: defeat Hardmode Samael. Hardmode charm gains: you no longer spawn enemy of your class on death.
Bonus quest: defeat Hardmode Deimoss at 100%. Hardmode charm gains: oskill instead of charges, but is class-specific.
Bonus quest: complete the 6 Trials without death and cube Hardmode Charm with corresponding emblem.
