So... Paradigm Shift only affects Moonstrike. So who the heck cares since Moonstrike is only used for the Buff anyways? No, Paradigm Shift affects the buff that Moonstrike adds. If you take Paradigm Shift you get 0 EWD from it.
Moonstrike used to have its own damage modifier but it was removed specifically to make this less confusing.

UPD: Tested with Miasma on Helm. In ~70% cases when miasma procs, enemy just don't take dmg from poison at all. Because Miasma low lvl and has very little dmg in compare to WayOfSpider. Seems spell and passive poison dmg just overwrite each other no matter which is higher oO.
Also tested Arachnomancy. When it procs, enemy looses health ~twice faster but the duration is twice shorter, so the dmg is almost same => again seems passive and spell poison dmg doesn't stack with each other. Anyway i will glad to see other comments.
They stack only on the first frame when you get a proc and hit at the same time. At any other time you'll just get one or the other poison at a time - whichever hit that frame, or whichever has the faster DPS afterwards (sounds like it's Arachnomancy in your situation).
Miasma is reducing your damage because it reapplies poison every frame, which stops the actual DoT that's on them from applying - the better DoT is still the one that sticks, but it doesn't do anything until they move off the Miasma.
