1) "on attack" : only the target you melee-attack can trigger the proc.
"on striking" : every weapon-based damage can trigger the proc.
example: Thunderslam from barbar. if you hit an enemy, you proc "on attack" and "on striking" and the weapon-based nova dmg, which hit the other targets around the target will proc "on striking"-effects but not "on attack". ranged attacks, like via bows, throwing weapons do not proc on attack
2) Melee/Ranged attacks and the other is weapon-based-damage (when the skill you use says: deals xx% weapon dmg) and basicly not weapon-based weapon dmg (mostly caster spells or summon spells)
3) It is instant but works only with weapon-based
physical dmg. Works like striking, so every weapon-based physical dmg will leech you life. But in hell you need a lot to notice a differnce. does work with ranged-weapons.
4) Combat Speed is: Attack Speed + Casting Speed + FHR + Faster Block (but not Faster Run/Walk) (like all res is: fire + cold +lighting + poisen but not physical)
